 When you place your order for one of the products below, you will be directed to the checkout screen, where you can enter your insurance information.

  Once you complete your order we will verify your insurance and determine how much if any you have to pay out of pocket for your items.   Before shipping your items we will need a prescription from your doctor, as well as any other forms your insurance may require.   After we receive your insurance information we will contact you to let you know if there is any additional forms your insurance may require.
